Registration

The following documents and information is required when registering a new student:

• A certified birth certificate.  (Kindergarten students must be five years old on or before September 1st of the current year.)
• A document showing proof of residence (such as a utility bill or lease agreement).
• Florida Immunization Form (Blue sheet from the Health Department or a Doctor).
• Physical (The physical cannot be over a year old.)
• Copy of the student's social security card.
